SLO Transit Senior/Disabled ID Card Application Forms

Senior-Disabled ID Card - Basic Application From (16kb)
This form is ONLY for those who are one of the following:
1. Age 65-79 years (Senior Discount pass)
2. A disabled U.S. veteran
3. Have a valid DMV placard or registration for a permanent disabled license plate or parking placard
4. Have a Medicare card, Social Security disability letter, proof of disability from the CA Dept. of Rehabilitation, ADA card, Braille Institute card, or a disability card issued by another transit agency.

Medical Certification form (28kb) - This form is for those applying for a discount pass for medical reasons, but do not meet the requirements of the above Senior-Disabled ID Card.
