Candidate's Guide for anyone considering running for one of the City's
three elective office seats in the November 2, 2010, general municipal
election.

candidates running for election throughout the county.

Below are State and City
mandated disclosure forms. Due to varying circumstances, some may not be
applicable to all candidates or committees. However, it is important
that all candidates, treasurers and committee principal officers
familiarize themselves with these forms, as failure to meet
disclosure requirements as specified by State Law can result it heavy
fines.
The Fair Political
Practices Commission (FPPC) has produced a manual to explain the purpose
of State disclosure forms, explaining who needs to file each form, and
when.

Candidates and
Treasurers are also recommended to attend a workshop at City Hall,
presented by the FPPC, on state form filing.

IMPORTANT FILING INFORMATION:

Original forms
submitted must have a "wet" signature, and copies filed must be of
the signed original.

State
forms include filing instructions. These are for your information only and
should not be submitted with the form.

Hard copies of these forms are available to campaign representatives
from the City Clerk's Office.

Candidate Statement of Intention -
Form 501
A Candidate Intention
Statement (Form 501) is requires to be files by any candidate for local office
prior to solicitation or receipt of any contribution, or expenditure of
any personal funds used for this election.

form with the City Clerk's Office.
Statement of Organization Recipient Committee - Form 410
File the Form 410 within 10 days of
receiving $1,000 in contributions.

File original signed
form and one copy with the Secretary of State (see the form instructions
for address), and one copy with the City Clerk's Office.
Recipient Committee Campaign Statement - Form 460
Form 460 is for
committees who have, or expect, contributions or expenditures totaling
$1000 or more. The form is used to report total contributions and expenditures, and
to disclose individual contribution of $100 or more. This form requires
all individual contributions of less than $100 be reported as a single
lump sum.

City Supplemental Campaign Contribution Statement
The City of San Luis
Obispo requires every monetary
contribution of $50 or more be individually disclosed. Use City Supplemental
form to report all contributions received by recipient committees of $50
or more, but less than $100, in supplement to Form 460.
Submit a copy of the City Supplemental form with each Form
460 submittal, completed for the corresponding contribution period.

page which remains blank after form has been completed, and number
remaining pages
accordingly in the space provided.

Campaign Statement - Short Form - Form 470
Form 460 is for
committees who have, or expect, contributions or expenditures totaling
less than $1000. The form is used to report total expenditures and contribution.

Late Independent Expenditure Report - Form 496
Use for reporting
independent expenditures that cumulatively total
$1,000 or more to support or oppose a single candidate or a single
ballot measure during the 16 days immediately preceding the election in
which the candidate or measure will be voted on must be reported as late
independent expenditures.

File Form 497 within 24
hours of receiving or making a monetary contribution or loan during the
16 days immediately preceding the election in which the recipient
candidate or ballot measure will be voted on.

Statement of Economic Interest - Form 700
Chapter 7 of the Political Reform Act
(Gov.
Code 87100-87500.1) requires every
mayoral and council candidate file a statement disclosing his or her
investments, business positions, interests in real property, and income
received during the immediately preceding 12 months.
